Welcome to TL Health Care Foundation

History

Dr. Su Kin Lee founded SK Health Foundation in 2016 with a clear and compassionate purpose: to serve patients suffering from severe and chronic illnesses. As a physician who had practiced medicine for decades, Dr. Lee witnessed firsthand how many chronically ill patients lacked coordinated and continuous care. Too often, this gap resulted in preventable emergency room visits and repeated hospitalizations. This year, Dr. Lee launched a post-discharge visit program with an IPA for medically fragile patients recently discharged from emergency rooms and hospitals. This program provides intensive follow up care directly in patients’ homes, ensuring continuity, close monitoring, and personalized support during the most vulnerable stages of recovery.

Recognizing this critical unmet need, Dr. Lee envisioned a model of care that would follow patients beyond hospital walls and address their complex medical needs in a more comprehensive and humane way.

In 2018, the organization was renamed TL Health Care Foundation, reflecting an expanded and enduring commitment to coordinated healthcare access for all.

Milestones

2025:

FQHC Look Alike: TL Health Care Foundation was approved as a Federally Qualified Health Center (FQHC) Look-Alike. Health Center Program Look-Alikes are community-based health care providers that meet all Health Center Program requirements. As an FQHC Look-Alike, TL Health Care Foundation provides comprehensive primary care services in underserved communities, offers care on a sliding fee scale based on patients’ ability to pay, and operates under a patient-majority governing board.

2026:

New Services Initiated: TL Health Care Foundation launched acupuncture and behavioral health services to expand its service scope and improve patients’ access to more comprehensive, integrated care.

Our Services

TL Health Care Foundation serves all patients—regardless of financial situation. We offer assistance to those who may have difficulty paying, including a discount program for eligible uninsured and underinsured patients. Your health is always our first concern.

We also uphold a strict nondiscrimination policy. Well woman examination: an annual, preventive check-up that focuses on a woman's reproductive and general health. It includes a health history review, breast exam, and often a pelvic exam or Pap smear to screen for cervical cancer.

Insurance Accepted

Medicare

Health insurance provided by the U.S. federal government, primarily for individuals aged 65 and older or those who qualify due to disabilities. It covers hospital care, outpatient services, and certain medical treatments.

Medicare HMO –Allied Pacific IPA

This is a managed care plan under Medicare (Medicare Advantage). Medical services are provided through a designated healthcare network. Patients are usually required to receive care within the network, and the plan may include additional benefits.

Medi-Cal HMO

A healthcare assistance program provided by the State of California for individuals who meet income eligibility requirements. Under the HMO model, patients must choose a primary care physician and obtain referrals to see specialists. Care is generally limited to providers within the designated network.

PPO

Preferred Provider Organization. Patients may receive care within the insurance company’s provider network and generally have more flexibility in choosing doctors. Referrals are usually not required to see specialists. However, out-of-network services typically involve higher out-of-pocket costs.

Discount Program

No one will be denied care due to inability to pay. We have a discount program that everyone can apply for. We will only ask for number of household members and household income to determine eligibility for the discounts.
