Welcome to TL Health Care Foundation

History

In 2016, Dr. Su Kin Lee founded the TL Health Care Foundation with a clear and compassionate purpose: to serve patients suffering from severe and chronic illnesses. As a physician with decades of experience, Dr. Lee witnessed how many chronically ill patients faced challenges due to a lack of coordinated community health services and continuous care. This gap often resulted in preventable emergency room visits and repeated hospitalizations. This year, Dr. Lee introduced a post-discharge visit program in partnership with an IPA, targeting medically fragile patients who have recently been discharged from emergency rooms and hospitals. This innovative program offers intensive follow-up care directly in patients' homes, ensuring continuity, close monitoring, and personalized support during their most vulnerable stages of recovery. Recognizing this critical unmet need, Dr. Lee envisioned a model of compassionate health care that would support patients beyond the confines of hospital walls, addressing their complex medical needs in a more comprehensive and humane manner. In 2018, the organization was renamed TL Health Care Foundation, symbolizing a strengthened and lasting commitment to coordinated healthcare access for all.

Milestones

2025: FQHC Look Alike: TL Health Care Foundation has been approved as a Federally Qualified Health Center (FQHC) Look-Alike. As a recognized provider of community health services, TL Health Care Foundation meets all Health Center Program requirements. With its designation as an FQHC Look-Alike, the organization is committed to delivering compassionate health care by offering comprehensive primary care services in underserved communities. They utilize a sliding fee scale based on patients’ ability to pay and operate under a patient-majority governing board. 2026: New Services Initiated: TL Health Care Foundation expanded its service offerings by launching acupuncture and behavioral health services. This initiative aims to enhance access to a more integrated, comprehensive approach to community health services for patients.

We provide compassionate health care assistance for individuals who may face challenges with medical expenses, and we have a discount program available for eligible uninsured and underinsured patients. Your health remains our utmost priority.

A well woman examination is an annual preventive check-up that emphasizes a woman's reproductive and general health. This essential part of community health services includes a comprehensive health history review, a breast exam, and often a pelvic exam or Pap smear to screen for cervical cancer. Medicare

Medicare HMO –Allied Pacific IPA

This is a Medicare Advantage managed care plan that emphasizes compassionate health care provided through a designated healthcare network. Patients typically need to receive care within this network, which also facilitates access to community health services. Furthermore, the plan may offer additional benefits, bolstered by the initiatives of the TL Health Care Foundation.

Medi-Cal HMO

The State of California offers a healthcare assistance program for individuals who meet income eligibility requirements, designed to connect residents with essential community health services. Operating under the HMO model, patients are required to select a primary care physician and obtain referrals to access specialists. Care is typically limited to providers within the designated network, ensuring residents receive compassionate health care as emphasized by the TL Health Care Foundation.

PPO

A Preferred Provider Organization allows patients to receive care within the insurance company’s provider network, which often includes vital community health services. This model generally provides more flexibility in choosing doctors; referrals are typically not needed to see specialists, facilitating access to compassionate health care. However, when patients opt for out-of-network services, they usually incur higher out-of-pocket costs. Organizations like the TL Health Care Foundation work to enhance access to these essential services.
